Working alongside Hospital Discharge staff, care homes, individuals, families & carers you will undertake care coordination, review and practical interventions within the Renfrewshire area to support discharge planning. This role is based within the Hospital Discharge team, as part of RHSCP Adult Services. Primarily based within Council Headquarters, however also within the RAH where required.
Responsibilities of the role include working closely with care homes to negotiate and plan hospital discharges in a timely manner, to support and promote the partnerships Delayed Discharge agenda. Experience of undertaking assessment and reviews for older adults within their own home or within a care environment is essential for this post.
You will operate under the Council’s WORKSMART arrangements which promote mobile and flexible work practices from any council premises travelling throughout the Council area.
You must have experience of managing within demanding and complex services and of working with customers/public and have experience of handling customer enquiries. You must also possess effective assessment and interpersonal skills & be able communicate effectively verbally and in writing.
You must be able to evidence an ability to develop effective working relationships with managers, staff, and other stakeholders from a variety of backgrounds and disciplines. Also demonstrating effective time management, ability to set priorities, follow processes, and must be task focused.
It is essential that you are able to use IT packages and have experience of updating & managing data through use of electronic systems. You must be able to work as part of a team as well as on your own initiative, prioritise your own workload, motivated, committed to Social Work values and be able to travel throughout the Renfrewshire Area.
An SVQ Level 3 in Social Care is essential or be willing to work towards. HNC in Health & Social Care is desirable as is the use of Eclipse.
Full training and support will be given to ensure that you are able to maintain the department’s commitment to continuous improvement.
