We are currently looking forregistered nurses with proven experience of working within a district nursingservice for the role of senior community nurse. This includes palliative care, wound care,catheter care. If you feel you have the skills and experience to be able tomanage a cohort of higher acuity/complex patient as directed by the team leader(caseload holder) we would welcome your application. Applicants will need todemonstrate excellent communication skills, and a sound ability to prioritise, timemanage their own patient visit list, complete complex holistic assessments,lead on developing plans of care and supporting/mentoring junior staff memberswithin in the service. Main duties of the job
The post holder will work as a senior member of the District NursingTeam collaborating with the Team Leader (caseload holder) and support theprovision of clinical leadership within the team. A key feature of the role isto support the proactive management of patients with complex or higher acuityconditions on the District Nursing Caseload. This will include collaboratingwith other members of the multi-disciplinary team involved in the patients careto establish a person-centred approach to management of health/social careneeds, that also promotes self-management. They will require the use ofextended clinical skills and expert knowledge to assess the physical and psycho social needs of a defined patient group, instigating therapeutic treatmentsbased on best available evidence in order to improve health outcomes. In doingso, they will assess, plan, deliver and evaluate patient centred programmes ofcare ensuring that they practice within their sphere of competence andknowledge. The post holder will work to professional and NMC codes, standardsand guidance at all times ensuring that their practice is based on the most upto date evidence-based theory and practical knowledge. They will be responsiblefor developing new and innovative clinical practices responding to emergingknowledge and techniques and working with the District Nurse Coordinator, TeamLeader and other, services, where appropriate to make improvements.

1.Clinical
Demonstrates adetailed theoretical and practical knowledge of common aetiologies,pathologies, conditions, and presentations involving a critical understandingof theories and principles.
Be proficientin core skills required to deliver care within a district nursing service suchas tissue viability, palliative/end of life care, diabetes management.
Demonstrateinnovative and specialist skills required for assessment and interpretation ofcomplex patient conditions and determining the appropriate course of action.
Select methodsand tools to holistically assess, plan, implement and evaluate in a complexpatient condition.
Analyse and actappropriately in complex situation and escalate wherever applicable, toDistrict Nurse Coordinator, Team leader, Safeguarding Lead, GP.
Make judgementson complex facts requiring interpretation and comparing treatment/managementoptions ensuring clinical reasoning is reflected accurately in case records.
