The role will involve working within the Primary Care and Community services team and the wider IHC, improving patient outcomes in relation to medicines management across all our services, ensuring the patient is at the centre of everything we do. As a member of our Medicines Management team, this exciting role will focus on Primary Care and Community Services, working to deliver a strong, joined-up service for patient safety and high quality sustainable care. A flexible and positive approach will be essential, in addition to excellent influencing, problem solving and time management skills. The post-holder will need to visible, be well organised and be able to facilitate and engage other to understand the benefits of the safe, effective and economic use of medicines. Main duties of the job The successful candidate will be a registered Pharmacy Technician who demonstrates excellent knowledge and understanding of medicines optimisation, safety and best practice in a primary care setting, as well as being able to manage interface issues with acute and community organisations. They will have outstanding interpretation skills and demonstrate the ability to communicate with a range of colleagues and health care professionals. Using a patient centered approach, elements of the role will include, providing; Pharmacy service across the IHC hospitals, GP practices and other clinical and non-clinical areas in the East area as needed. Undertaking, medicines reconciliation, medication audits, supporting structured medication reviews and quality improvement work.
