The East Lancashire Alliance is currently looking to recruit a care coordinator on behalf of the Rossendale East & West PCN's.
The role is for 30 hours per week. Please note the pay is pro-rata.
A keen eye to detail and flexibility is key in this role, and whilst the role will predominantly be based at Haslingden Health Centre, this role will involve working across practices in Rossendale, so own transport is necessary.
Main duties of the job
The post holder will be responsible for the provision of wide-ranging and efficient administrative support to the Rossendale East & West Primary Care Network (PCN). Typically, this support may include the arrangement of meetings, dealing with correspondence, document management, preparing short reports, creating spreadsheets, responding to and forwarding e-mails, and other administrative tasks such as minute taking of PCN meetings.

Job responsibilities
You will work closely with clinical pharmacists, advanced clinical practitioners, doctors, nurses, healthcare assistants, administrative staff, and management teams to ensure patients are being directed to and receiving the most suitable care for them.
Duties and responsibilities include:
1. Understand what a Primary Care Network is and how support is provided to patients due to improved collaboration between health and social care services.
2. Work with key people in the PCN to develop & support collective general practice projects including areas of federated working.
3. The post holder should demonstrate good organization and time management skills.
4. Always maintain confidentiality.
Communication and Key Working Relationships
1. Demonstrates ability to work as a member of a team.
2. Recognize personal limitations and refer to more appropriate colleague(s) when necessary.
3. Actively work toward developing and maintaining effective working relationships both within and outside the PCN or group of PCNs.
4. Liaise with other stakeholders as needed for the collective benefit of patients including but not limited to Patients GP, Nurses, other practice staff, other healthcare professionals, and commissioning organisations.
5. Act as a point of contact when required for people who wish to contact the PCN.
6. Communicate effectively with service users and their families/carers, and other staff both internal and external and members of the public.
7. Respond positively, in a courteous and professional manner when dealing with routine and non-routine enquiries which may be complex and involve problems in communication/understanding.
8. Communicate complex and highly sensitive information which may relate to staff, patients, and relatives or commercial undertakings, within and outside the organization, with the ability to persuade, negotiate, and influence others.
9. Help maintain excellent communication, liaison, and working arrangements with Trust Directorates, Trusts, NHS England, East Lancashire Integrated Care Board, Social Services, Voluntary Sector Organisations, Care Homes, and other agencies.
10. Develop excellent working relationships with partners and wider service networks, including the voluntary sector, GP practices, adult social care, hospitals, community pharmacists, and other key PCN stakeholders.
11. Meet regularly with the clinical & management leads of the Direct Enhanced Services.
12. Manage and prioritize workload on a daily basis and deal with the competing demands of PCN Admin work.
Planning and Organisation
1. Manage and prioritize all incoming and outgoing correspondence, initiating responses where appropriate.
2. Manage the diaries of key members of the team when required including maintaining and coordinating appointments, organizing meetings (including videoconferences via Microsoft Teams), and using initiative to make necessary revisions.
3. Prepare agendas, collate and circulate papers for meetings, ensuring that all papers are submitted in time and comply with the required house style standards.
4. Select, retrieve, and collate papers and information in a timely manner for meetings and in response to enquiries.
5. Attend meetings, taking and transcribing formal minutes and action notes.
6. Manage the organization of events or meetings which involves sending invites, arranging speakers/lecturers, and booking venues.
7. Organize and prioritize workload and work flexibly in response to competing demands.
Responsibility for Patient/Client Care, Treatment & Therapy
1. Support the process of holistically bringing together all of a person's identified care and support needs and explore options to meet these within a single personalized care and support plan (PCSP).
2. Support the process of helping patients to manage their needs through answering queries, making and managing appointments, and ensuring that patients have good quality written or verbal information to help them make choices about their care.
3. Support the process of patients being able to take up training and employment and to access appropriate benefits where eligible.
4. Assist the process for patients to access self-management education courses, peer support, or interventions that support them in their health and wellbeing.
5. Support the process of patients being able to access personal health budgets where appropriate.
6. Provide coordination and navigation for patients and their carers across health and care services.
7. Effectively use all methods of communication and manage barriers to communication.
8. Support the process that provides information to patients, their carers, and/or relatives on behalf of the team.
9. Be the point of liaison for service users and interface with all health and social care professionals.
10. Receive and collate information in connection with the PCN workstreams.
11. Use risk stratification tools provided and support presentation information in review meetings.
12. Follow through actions identified in the PCN work streams including arranging tests, referrals, signposting, etc.
Responsibility for Supervision, Leadership and Management
1. Assist the induction of new members of the team.
2. Provide cover for colleagues during periods of absence and leave, as required in the interest of the service.
3. Adapt to a rapidly changing environment and encourage team members to embrace necessary adjustments.
4. Have a positive attitude to challenges and their solutions.
5. Work with the GPs and other primary care professionals within the PCN to identify and manage a caseload of patients.
6. Raise awareness within the PCN of shared decision making & decision support tools.
Information Resources and Administrative Duties
1. Prepare reports for submission, produce letters, presentations, and other documentation from audio or manuscript to a professional standard.
2. Take minutes of formal meetings and disseminate; chase progress against actions identified in these meetings.
3. Maintain strict confidentiality relating to PCN business at all times.
4. Develop and maintain spreadsheets and databases/computerized appointment systems/information systems and internal forms for use across the service.
5. Assist other staff at all levels in the use and development of office equipment and specific IT applications.
6. Maintain professional skills and competencies and undertake any necessary skills training, professional updates, and mandatory training as appropriate to the post.

Qualifications

* NVQ Level 3 in adult care advanced level or equivalent qualifications or working towards

* Degree level education or equivalent
