You will work collaboratively with them, your colleagues, and external providers to promote recovery. Supporting service users to successfully transition from an inpatient setting to the community and assist them in maintaining their stability within their community environment, helping them to build their own support networks. Main duties of the job Work as part of a team that provides specialised mental health services to individuals with complex needs in a community setting. The focus is on addressing the direct needs of these service users while collaborating across different care boundaries, organisations, and roles. These individuals are clients of specialised services in Secure and Complex Care and may be subject to a Community Treatment Order or conditional discharge under the supervision of the Ministry of Justice. The successful candidate will: Engage service users through in-reach into the hospital before their discharge into the community, and continue to work with them in the community to promote recovery. Develop services and establish activities for both groups and individual service users that align with their specific needs and recovery goals. Support service users in identifying their needs and assist care coordinators in planning, implementing, and evaluating individual care plans. Prioritise the individual needs of service users at all times, working within a recovery model to help them achieve their identified goals.
