Job summary The Sutton Virtual Community funded by the Health Foundation as part of the Tech for Better Care fund is seeking a Patient Engagement and Service Manager to lead our efforts in fostering strong relationships with patients and enhancing their experience within our innovative digital health platform. This role is crucial in ensuring that our services are user-centric and effectively address the needs of individuals managing chronic conditions, particularly loneliness and other psychosocial factors. Main duties of the job Patient Engagement Develop and implement strategies for engaging patients and community members in the Sutton Virtual Community platform. Build relationships with patients, advocacy groups, and local community organisations to encourage participation and feedback. Organise and facilitate focus groups, workshops, and educational sessions to gather insights and enhance user experience. Service Management: Oversee the day-to-day operations of patient engagement services, ensuring they align with the projects objectives and standards. Collaborate with healthcare professionals (GP Practices), support groups, and digital platform users to improve service delivery and address issues promptly. Monitor and evaluate service effectiveness, using data and feedback to inform improvements and adjustments to the programme. Work closely with our tech partner Together.ly to relay patient feedback, explore solutions and implement changes. Ensuring data privacy and compliance with health data regulations (e.g. GDPR) when working with patient information.
