Newcastle North Primary Care Network (PCN)
Thecare coordinator will assist each of the member practices within the PCN intheir collaborative work. The post-holder will work closely with the ClinicalDirector, Digital and Transformaton Lead, Operations Manager, PCN Administrator and Practice Managersacross the network.
There are 3 care coordinators within the PCN, workingwith each GP practice. It is expected that the care coordinators will work as ateam to share good practice and on occasion provide some cross cover.
Main duties of the job
The main requirements of this role will be acombination of identifying and understanding specific patient cohorts, patientengagement and administrative tasks that support the smooth and effectiverunning of the PCN.
Though the role is predominantlyorganisational, there will be a requirement to contact patients (via text,letter or telephone) for a variety of reasons such as booking for vaccinations,arranging health screening or gathering data.
As a highly motivated individual, the postholder will be expected to provide high quality co-ordination support across arange of services and projects delivered by Newcastle North PCN. This is afast-paced environment that requires excellent organisational skills.

Newcastle North PCN comprises of 5 separate member GP Practices in 4 locations working together to provide patient care to a population of just over 35,000.

Job responsibilities
Thesuccessful candidate will have experience of EMIS web or similar patientmanagement software and Excel spreadsheet knowledge for regular reporting.Other duties will also include;
Supporting practices to enhance care; thiswill include supporting practices to achieve PCN Network DES targets
Carry out audits to identify improvements andmaximise income in particular in Investment and Impact Fund (IIF), QOF, QIF, enhancedservices and claiming procedures
Update IT systems to include templates andprotocols in order to meet the requirements of any changes in servicespecifications
To coordinate, monitor and deliver projectsand services clinical campaigns
Supporting practices to achieve the publichealth indicators linked with PCN quality improvement work e.g. cancerscreening uptake
Supporting the PCN vaccination campaigndelivery (Covid and Influenza). This will involve running searches, monitoringtargets and contacting patients.
To support the wider PCN in developingeffective working relations which maximise health outcomes for the PCN patientcohort
IT and Administrative Support:
Linking with the PCN operations manager and PCNadministrator to ensure appropriate searches developed and kept up to date tomonitor progress
Using PCN dashboards to monitor progress
Having a good level of understanding of EMIS,Ardens and other tools that can support highly effective working across thePCN; develop appropriate EMIS searches to support work streams as necessary
PROJECT ADMINISTRATION:
NewcastleNorth PCN is constantly looking to develop new services to meet the needs of thePCN contract and our patient population. This role will offer you theopportunity to get involved in these developments. We would like to use your organisationalskills to deliver services on time, to the highest standard.
Support the general administrationof projects and services delivered by our PCN.
Support functions coveringplanning, tracking, reporting, quality management and internal communication.
Support the production of projectperformance reports for the Board.

Qualifications

* Willingness to work flexible hours when required to meet work demands
* Access to own transport

Experience
