An opportunity has arisen for a Trauma/Orthopaedic Discharge Coordinator to join our team based at Stoke Mandeville Hospital.
As Ward Discharge Coordinator, the post holder will provide support to facilitate and drive the safe, effective, and timely transfer of patients from the Trust to the most appropriate setting to meet their needs, reducing length of stay and maximizing the use of hospital beds.
The post holder will ensure best practice in the provision of a discharge service within the Trauma/Orthopaedic wards through the stipulation of a high-quality, consistent, robust, proactive, and timely service.
Responsibilities include:
1. Providing a single point of communication with patients, relatives, and carers, ensuring that the discharge processes are patient-centered and consider their needs and wishes.
2. Managing an individual caseload while working with support and supervision from the complex discharge coordinators when necessary.
