Job Purpose - Monday to Friday
The post holder will work autonomously and in collaboration with other health care professionals, statutory and voluntary agencies, and the Planned Care locality teams to deliver excellent standards of patient care for patients requiring support in unplanned care.
Responsibilities:
1. Deliver high quality, skilled clinical care to patients at home and provide rapid response, rehabilitative and post-acute care in a variety of community settings including weekends and bank holidays. The service operates through pre-rostered shift patterns spanning 8am - 10pm 7 days a week.
2. Utilise specialist skills and knowledge to holistically assess needs and provide short term programmes of nursing care for vulnerable adults based on evidence-based practice.
3. Work, as required, on a flexible basis to cover all roles required within the team including in-reach support at Watford General Hospital.
4. Work within and across professional and organisational boundaries, liaising with GP's other professionals and statutory and voluntary agencies to deliver individualised and expert clinical care to a high standard to clients.
5. Take responsibility for managing the caseload and the team, with the support of the Senior Nurses.
6. Support the Locality Manager/Clinical Lead and colleagues in delivering high quality care that delivers best value, and meets the requirements of commissioners.
Main duties of the job
1. Provide skilled, effective, and evidence-based clinical care to patients in a variety of community settings.
2. Use enhanced skills and knowledge to assess physical and psycho-social needs and develop a comprehensive and concordant plan of care with patients, carers, and the wider Multidisciplinary Team as needed.
3. Make risk assessments, establishing the safety or otherwise of patients' ability to stay at home, or return home safely from another care setting, providing and arranging necessary nursing and supportive packages.
4. Provide autonomous clinical care to a range of patients presenting with undifferentiated, undiagnosed and/or complex health needs and long-term conditions.
5. Support appropriate and timely case-finding of patients suitable for the service including working to support in-reach to local acute trusts as needed.
6. Provide evidenced-based clinical/therapeutic interventions based on 'best practice', in order to improve health outcomes and promote choice, e.g. Intravenous therapy, anticoagulation monitoring, palliative care interventions, and catheter and bowel management.

Education/Qualification
Essential
* Registered Nurse with current NMC registration OR Paramedic with HCPC registration.
Desirable
* Experience of working in Primary Care/ Accident & Emergency.
* Experience of supervising junior staff and coordinating daily activities on a regular basis.
* Physical Clinical Assessment Skills Course or willingness to undertake the course.
Experience

* Extensive range of clinical skills and experience including, e.g. IV therapy, venepuncture, cannulation, rehabilitation.
* Experience of undertaking comprehensive health needs assessment and writing care plans.

* High level of competence in a range of clinical skills.
* Able to work without direct supervision, planning and prioritising own workload and that of others.
* Basic IT skills including use of Microsoft Office package and email.

* Principles of care for patients with long-term conditions, relevant NSFs and other national standards.
* NMC/HCPC Codes of Professional Conduct and application to practice.
