Non-clinical Care Coordinator. This is a 1-year fixed-term contract with the possibility of extension depending on ongoing funding.
This non-clinical role's overall purpose is to facilitate safe and timely case management and discharges of patients suffering from gambling harm and those affected by gambling harm. The successful candidate will work closely with the Mental Health Nursing Team, supporting the nurses and holding a small caseload.
Main Duties of the Job
The complexity of patients will vary and require differing levels of support and facilitation; however, the purpose of appropriately discharging the patients to a suitable environment will remain the focus of the role.

The Primary Care Gambling Service (PCGS) is now an established service across London with the potential for expansion.
Job Responsibilities
The post holder will be required to:
1. Facilitate safe and timely discharges ensuring for patients suffering from gambling harm and their affected others.
2. Support the MH Nursing to achieve a collaborative approach to discharge.
3. Ensure that the patient understands post-discharge plans and arrangements for continuing care such as onward care with the third sector.
4. Coordinate all appropriate procedures working in partnership with GPs, MH nurses, therapists, and outside agencies, ensuring that all plans are in place for the time of discharge.
5. Complete all documentation pertaining to onward referral.
6. Be responsible for the effective coordination of patient discharges throughout the PCGS.
7. Escalate issues to the MH Team Leader as needed.
8. Work closely with the MH nursing team, NHS organisations, GP and secondary care, the third sector, and wider multi-disciplinary team members to deliver a high-quality patient-focused service.
9. Act as the key liaison person between the 3rd sector and patients' onward care.
10. Attend weekly discharge meetings when required, ensuring that discharges are effectively communicated and planned, completing all clinical records and facilitating the completion of any relevant actions/documents from such meetings.

Qualifications
* Experience of discharge planning.
* Evidence of continuing professional development.
* Work experience in management and leadership roles.
* Educated to GCE/GCSE standard.
* RSA Stage II/NVQ Level 2 in Administration or equivalent.
* Good command of written English.
* Experience of successfully liaising with multi-disciplinary teams, including welfare, performance, and development experience of creating reports, and project plans.
* Experience of participating in service development and managing projects.
* Experience of working with a range of busy professionals in a changing environment.
* A clear understanding of gambling-related harm and the effects on family members and the wider community.
* Demonstrate understanding of patient/client confidentiality.
* Experience in delivering excellent customer care.
