An exciting development opportunity has arisen for a Band 7 Team Lead to work within the Virtual Clinical Care Team.
The successful candidate will need to demonstrate significant experience in the field of Community Nursing with skills in virtual assessment, together with a genuine desire to develop both the service and themselves.
The Team Lead will work in partnership with the Matron and ICT services to ensure that the highest possible standards of practice and service delivery are maintained. As part of this, the post-holder will have a key role in managing clinical and non-clinical operational systems and processes within the team.
They will use their management experience to lead on service changes and developments and contribute at a local level to the development and implementation of policies and procedures.
They will lead a team, with involvement in service coordination, staff wellbeing, supervision, complaints, governance, and service performance and improvement.
Main duties of the job
The service aims to provide an excellent standard of person-centered assessments to patients as part of an integrated service that supports patients to remain in their home environment. You will work closely with the multi-disciplinary team in order to support the service and have a high focus on delivering quality outcomes and improving the patient experience.
You will be expected to assess patients within a given geographic area and may need to liaise with a variety of GP surgeries.
As both an organization and a service, we are actively involved in both local and national innovative projects around improving the quality of care, integrated working with stakeholders, and designing new pathways. We have an extensive program of training and skills development available for staff and actively nominate and encourage our staff to apply for national awards and recognition, such as Queen's Nursing Institute, RCN, and Nursing Times awards.
The service runs from 7:30 am to 11:30 pm, 7 days a week, including Bank Holidays.

* NMC Registration - Registered 1st level nurse or HCPC Paramedic Registration
* Recent external CPD, post-qualification.
Desirable
* Specialist courses appropriate to Primary care e.g. CHD, Diabetes, Respiratory conditions, minor illness, and minor injury.
* Complex Case Management (LTC) qualification or experience
* Relevant postgraduate study at master's level
* Unwell Adult Clinical Skills Qualification

Experience / Knowledge

* Evidence of relevant experience working in a healthcare setting at senior clinical level
