The post is fully based at Ealing site, however there may be some occasions where the post holder is expected to visit the other sites for meetings, training, etc. Main duties of the job To lead, deliver, develop and evaluate general clinical pharmacy services in accordance with the objectives and priorities agreed between the post holder, Chief Pharmacist and Clinical Services Manager in order to ensure safe, clinically effective, and cost efficient use of medicines. To be a valued member of the clinical pharmacy team, work closely with the multi-disciplinary teams to provide a consistently highly specialist pharmaceutical service to patients on acutewards as well as palliative patients within Meadow House Hospice. To provide a consistently highly pharmaceutical service for patients within a range of areas within the acute hospital wherever needed as well as being a link pharmacist for Meadow House Hospice in line with Trust and national standards, in order to meet all Clinical Governance requirements To be able to work as part of a multidisciplinary team within the department and wider teams to ensure optimal medicines usage and value for money. To provide a framework for the safe, efficient and cost effective use of medicines within the Trust, by developing procedures, policies and guidelines, and participating in clinical audit. To participate in multidisciplinary ward-based education and training of junior doctors, pharmacists and nursing staff. To provide daily ward visits to acute wards as well as Meadow House Hospice, inputting on appropriate guidelines and attend MDT ward rounds. To provide regular support to the dispensary.
