Aim of the role:
As an essential member of our Homecare services team, your work will be closely aligned with the Clinical Lead Nurse as well as our managers to ensure the highest standard of person-centred care. The focus will be to ensure the service is of the highest quality by supporting the Clinical Lead Nurse in implementing and monitoring the clinical and management strategy for the homecare.
Key responsibilities will encompass:
* Conducting initial assessments of service users, developing individualized care plans in collaboration with patients, families, and healthcare professionals.
* Ongoing care plan reviews and risk assessments.
* Observing care staff and ensuring their competency.
* Managing clinical paperwork, including Medication Administration Records (MARs) – monthly audit.
* Providing general clinical oversight of the branch and service users.
* Occasionally providing direct care to service users to maintain hands-on clinical skills (e.g., tracheostomy change/sign off carer competency at the same time).
Main duties:
* Shadowing & sign off competencies for candidates.
* Initial assessments & implement care plan for individual service users.
* Care plan & RA reviews – shared task between Community and Clinical Lead Nurse.
* Medication audit – monthly, via Birdie.
* Quarterly spot checks.
* Building rapport/regular check-ins with parents – establishing effective working relationships with parents, their families, and carers. This will include promoting individual rights and recognizing and respecting their contributions to care planning and delivery.
* Working with a multi-disciplinary team.
* Ensuring admin is up to date.
* Working closely with the Clinical Lead Nurse to ensure care delivery is of a high quality and meets required standards.
* Supporting and deputizing for the Clinical Lead Nurse and providing knowledge and skills to support care delivery.
