We have 36,000 patients and all practices are situated in Sheffield 12 and Sheffield 13.

JOB SUMMARY

The post holder must be a prescriber.
The post holderwill be undertaking structured medication reviews, improving medicineoptimisation and safety,supporting care homes,as well as running practiceclinics, as set outin the national service specification structured medication reviews in the GPcontract. You will also be actioningacute prescription requests and conducting medicines reconciliation.
KEY RESPONSIBILITIES
The following arethe core responsibilities of the PCN Clinical Pharmacist. There may be, on occasion, a requirement to carry out other tasks.This will be dependent upon factors such asworkload and staffing levels:
To work as part of a multi-disciplinary team whilst in a supporting function to the PharmacyTeam and other clinicians and within agreed procedures
Undertake medication reviews to support improvedpatient outcomes including:
increased medication adherence
stopping medications that are causing harm,
Participate in pharmacy meetings, andother meetings to improve engagement and to promote issues relevant toprescribing and medicines optimisation.
Support the Medicines Optimisationteam in the delivery of the Medicines Optimisation strategy in Sheffield.
Handle function specific information, which may be sensitive, complex or confidential and appropriatelyrecording, transferring and/or coordinating such information in accordance withthe Data Protection Act; Caldecott Guidelines and the Confidentiality Code ofConduct
Beresponsible for the organisation, planningand of own workload to meet setdeadlines.
Utilise GP practice clinicalsystems to conductprescribing audits SystmOne/Emis.
Ensure that all major MHRA drug alerts,where appropriate and within levelsof competence, are acted upon within practicesover an appropriate timeframe and to advise on any medication changes that occur as a result.
Undertake training and development, toensure the required knowledge and skills for practice-based work.
Carry out other duties which areappropriate to the skills and competencies of the post holder and grade.
Establish and maintain effectiveworking relationships with GP practices, CCG and other healthcareprofessionals/stakeholders across the PCN.
Provide direct support and advice to GP practices and health professionals to promote and implement high quality prescribing tothe benefit of patients.
Provide peer support to other clinicalpharmacists in the PCN
