As part of ongoing development and expansion, we are developing our Hospital Liaison Team based at Doncaster Royal Infirmary. The team is an all-age service and we have older adult and substance misuse staff embedded within the core team.
We welcome applications from individuals with a minimum of 2 years experience delivering high-quality assessments alongside evidence-based interventions to adults receiving care within our local acute hospital.
This post is an exciting opportunity to make a difference working in a developing service at the front end of physical and mental health care in the acute hospital setting. This role will enable you to make a real difference to the lives of local people by offering assessment, intervention, and liaison in relation to co-existing mental health and psychological issues. The service will work closely with physical health, community mental health, and IAPT services as well as our local social care and non-statutory colleagues.
The role calls for a highly skilled individual with excellent assessment, communication, and brief intervention skills. The successful candidate must enjoy working as part of a wider team and fully understand the importance of liaison with other services. An awareness of how physical and mental illness can impact outcomes and quality of life is a must in this position, as well as good negotiation and problem-solving skills.
Main duties of the job
1. To understand the requirements of the Mental Health Act 1983, Mental Capacity Act 2005, Care Act 2014, Care Programme Approach, and Deprivation of Liberty safeguards as it applies to a psychiatric liaison service.
2. To promote and assist in the maintenance of the highest attainable standard of nursing care in a general hospital setting, drawing on examples of current best practice.
3. To provide specialist support and advice to referring agencies and case management, preventing unnecessary delays.
4. To maintain timely clinical records and share content of clinical records/information appropriately with other professionals and agencies, working across interfaces to improve communication and timely patient care.
5. To advise patients, carers, and significant others of the available resources, referring assessed need to those agencies best able to meet these in the community and facilitate the use of these resources.

Qualifications/Skills
* Registered Mental Health Nurse.
* Appropriate demonstrable experience of working with people with mental health problems. Experience of working with carers and families.
* Demonstrates advanced and comprehensive communication skills. Clear and concise command of English (written/oral). Ability to organise and carry out teaching practice. Basic IT skills/Computer skills. Positive presentation skills.
* Self-motivated, starter and finisher. Must be able to demonstrate the ability to prioritise workload. Organised. Highly motivated. Flexibility.
* Evidence of ability to work across team and organisational boundaries developing multiprofessional and multiagency partnerships. Hospital liaison experience. Experience of integrated working with physical health services.
* Excellent knowledge of mental illness, signs, and symptoms.
