The Stoke on Trent 0-19 Integrated Health Visitor and School Nurse Service has a vacancy for a dynamic, energetic and forward thinking Band 4 Children, Young People and Families Practitioner working 30 hours per week based at Smallthorne Health Centre., The 0-19 Children's, Young People and Families Practitioner will work within uni/multi-disciplinary teams in addressing a range of care and support issues across a spectrum of needs. They will contribute to the delivery of the Healthy Child Programme (0-19) and to the improvement of the health and wellbeing of the population served. The nature of the service will be determined by local service specifications.The 0-19 Children, Young Peoples and Families Practitioner will work in a variety of settings including clinics, Family Hubs and schools, providing advice, information, guidance, signposting and referrals. In return you will be professionally supported and empowered to deliver a child friendly service. You must have a Nursery Nurse Qualification or equivalent for this role (see Job Description).
