Job overview
An exciting opportunity has arisen for a Band 5 Physiotherapist to join the St Helens Early Supported Discharge team. The service provides stroke rehabilitation to the people of St Helens in their own homes in order to gain function and independence after a stroke.
Additional funding has been agreed for the next 12 months to expand and develop the service on offer. The successful candidate will be involved with supporting this pilot project giving plenty of opportunity to develop neuro and leadership skills alike. In the event that funding is not continued after the initial pilot, the successful candidate will be offered a rotational post within the organisation.
This is a community-based post, and the ability to travel between visits is essential.
Main duties of the job
The post holder will be expected to effectively manage a caseload of patients and deliver rehabilitation programmes as an autonomous practitioner, using evidence-based patient-centred principles to assess, plan, implement and evaluate interventions of the patient group.
The post holder will take a holistic approach in the assessment and treatment of patients, being responsible for the provision of appropriate, high-quality therapy and care to a caseload which may be of a complex nature, using the appropriate model of care and evidenced-based practice.

Detailed job description and main responsibilities
Job Purpose:
The post holder will be expected to effectively manage a caseload of patients' co-morbidities as an autonomous practitioner, using evidence-based/patient-centred principles to assess, plan, implement and evaluate interventions of the patient group. The post holder will provide leadership for junior staff, when appropriate, through supervision and appraisal. The post holder will participate in planning, development and evaluation of Physiotherapy services within a designated area/team, holding responsibility for defined projects.
The post holder will take a holistic approach in the assessment and treatment of service users/patients, being responsible for the provision of appropriate, high-quality care to a defined caseload which may be of a complex nature, using the appropriate model of care and evidenced-based practice.
Work in partnership with other professionals to enable patients/clients to be maintained in an appropriate environment, thus avoiding hospital admission.
To contribute to the maintenance and development of Physiotherapy Profession within the service and will facilitate effective learning within the area of practice for all students and practitioners.

Qualifications
* Registered Physiotherapist with the Health Professions Council
* Diploma/Degree in Physiotherapy
* Enhanced DBS Disclosure
* Teaching/Mentorship qualification or equivalent work-based experience
* Membership of professional Body
* Evidence of post-registration study in a field relevant to post
Knowledge
* Experience of Physiotherapy functional assessment
* Experience of successful team working
* Experience of liaison with internal and external services and agencies
* Knowledge and ability to express an understanding of Physiotherapy process including formulation of intervention plans grounded in clinical reasoning
