Job summary Our service is expanding and we are offering an exciting opportunity to join our busy Integrated Care Coordination for Urgent Care Team as a Band 6 NMC / HCPC Registered Triage Nurse / Paramedic based in Hanley, Stoke-on-Trent. We are looking for a passionate and dedicated Triage Nurse / Paramedic to provide triage assessment, clinical support, assistance and professional expertise to all health and social care registered professionals requiring assistance and support with Admission Avoidance which may range from signposting the caller to the appropriate service, making a direct referral to another service or offering immediate clinical advice or treatment. The service receives a high number of requests each day, for patients across Staffordshire and Stoke on Trent and you will have responsibility of managing a busy clinical triage portal and telephone line. You work as an integral member of the clinical MDT team to promote seamless patient care providing a quality service, aspiring to achieve, share a corporate pride in achievements and strive to act as an ambassador for good quality care. You will need to be flexible, approachable and have a sound clinical evidence based background and knowledge. You will need to be friendly but assertive, and be confident in own decision making, being able to challenge when necessary. You will be required to cover the 7 day period, 365 days and therefore weekend and out of hours working is required. Main duties of the job As part of this role you will be expected to: Work as a key member of the Integrated Care Coordination Team to ensure professionals are able to make referrals through one access point. Offer sound clinical advice. Work in conjunction with the Clinical Team Lead to provide high quality, clinical triage using knowledge, skills and critical thinking and professional judgement. Control and manage calls effectively by using skilled questioning, active listening and excellent verbal communication skills in a calm, professional manner. Identify and report any case of suspected or confirmed abuse to an individual, in accordance with agreed guidance and procedure. Work as a member of the clinical MDT team to promote seamless patient care. Be personally accountable for professional and ethical actions and omissions in compliance with the NMC / HCPC Code of Conduct. Be prepared to work flexibly, in the interests of self-development and pressures of the service. To actively participate in the on-going development of the role and support the development of the protocols and processes within the service.
