Role Overview: We are seeking a talented and motivated Clinical Lead Nurse (equivalent Band 6) to lead the delivery of complex care into the home. This role will involve managing and overseeing care packages in your region, working closely with the Senior Nurse Manager, Registered Manager, and Head of Clinical Services to provide leadership and ensure high standards of care. You will have the opportunity to develop personalised, client-centred service outcomes, carry out care assessments, write care plans, and manage competencies. As Clinical Lead Nurse, you will play a pivotal role in ensuring that our service users receive the highest standard of care. You will also be responsible for supporting, supervising, and developing our nursing and care teams to achieve outstanding results. Key Responsibilities: Clinical Leadership and Care Delivery: Develop individualised care plans in partnership with the service user, family, and other healthcare professionals, ensuring goals are patient-focused, realistic, and achievable. Monitor client and family-centred outcomes, using your clinical expertise to guide and implement necessary changes. Undertake initial holistic nursing assessments in collaboration with the Service User, family, and carers, involving statutory/voluntary/third-party sector providers as needed. Oversee and manage the day-to-day care delivery within your region, ensuring all standards and policies are upheld. Ensure all recording systems and organisational documentation meet required standards and are up to date. Supervise and support care teams through regular supervisions, competency assessments, and appraisals. Team Management and Development: Lead and motivate the nursing and care team, embedding the company's values and fostering a culture of pride in their work. Manage field-based care support workers and nurses, ensuring they are trained, assessed, and signed off as competent before working independently. Develop and implement action plans to improve service quality, client outcomes, and business development. Support and coach the team, working closely with Central Support, including HR, to ensure staff are well-supported and perform to the best of their ability. Quality and Compliance: Maintain a strong commitment to quality standards, ensuring compliance with statutory, mandatory, and contractual requirements. Ensure service user welfare is promoted and protected at all times. Maintain your professional skills and undertake continuous training for your personal and professional development. Follow national standards, legislative requirements, relevant regulations, and best practices in all areas of care delivery. Collaboration and Communication: Work closely with multidisciplinary teams to maximise opportunities for service users and ensure holistic care. Report against targets and service standards, providing feedback and sharing ideas for improvement with the senior management team. Build effective working relationships with key stakeholders, including case managers, healthcare professionals, and government bodies. Skills and Experience: We are looking for a Clinical Lead Nurse who is passionate about combining clinical expertise with leadership and management skills. You will have: Previous experience in delivering care for individuals with Spinal Injury, Acquired Brain Injuries, and Cerebral Palsy is essential. Experience in community nursing, with the ability to work across both adult and paediatric care. Professional nursing registration with no restrictions. Strong knowledge of and commitment to quality standards and compliance with statutory, mandatory, and contractual requirements. Ability to develop effective working relationships with stakeholders such as case managers, integrated care systems, GPs, consultants, and multidisciplinary teams. Proficiency in IT and excellent communication skills. A full UK driving licence and the ability to travel within the region.
