Job overview
An exciting opportunity has arisen to join our multidisciplinary community team as an Unplanned Community Sister/ Charge Nurse. This includes coordinating and delegating individual patient care by working across boundaries within secondary and social care settings.
As the unplanned care team, we cover the whole borough of Doncaster and provide all aspects of nursing care to patients in their own homes. Providing clinical care for patients with complex health needs who are at risk of deterioration which may result in declining quality of life, avoidable hospital admissions, or an unnecessary length of hospital stay.
The aim of the service is to promote self-care of long term conditions, avoid hospital admissions and support timely hospital discharges to maximise individual potential for independent living within a person’s usual place of residence. Working in an unplanned and out of hour’s capacity 24 hours, 7 days per week.
We are seeking to appoint a Sister/Charge Nurse who can demonstrate the vision and skills to work innovatively, flexibly and in partnership with other agencies supporting the MDT team assessment and provision of care to the high standard we aim for. The Sister/Charge Nurse is responsible for the day-to-day management of the team, as well as clinical decision making.
Main duties of the job
The role will rotate between Leadership and Management, Shift Co-ordination, and assessment of the complex clinical patients within the Unplanned Care Team. This role involves assessing & giving advice over the telephone and face to face, utilising excellent communication skills, IT skills, clinical expertise, telephonic clinical assessment, diplomacy skills and judgement to ensure that referrals into Community Nursing are managed effectively and the relevant services are involved within appropriate timeframes.
The role will include assessment of complex patients within their own home. A multi system’s approach to case management will also be required, linking in with case managers and the wider health and social care community, to ensure a seamless patient journey for a wide variety of care needs across the Doncaster area. The first line management of the clinical staff within the team, offering support, mentorship, acting as a positive role model, having a solution focused ‘can do’ approach to any challenge, ensuring that standards are maintained and the best practice evidence base is adhered to.
