Job Title: Social Worker – Sussex – Hospital & D2A Team
Location : Sussex
Rate : Up to £38 per hour
An exciting job role has recently become available in East Sussex. They are looking for a new member for their Hospital & D2A Team Discharge Team.
What will your responsibilities be?
As an Adult Social Worker for the Hospital Discharge Team, you will conduct thorough assessments under the Care Act to determine patients’ needs and support requirements, coordinate services and resources to ensure smooth transitions for patients leaving the hospital, and demonstrate expertise in adult social care legislation and practices to provide effective support and advocacy. You will utilize strong written and verbal communication skills to interact with patients, families, and other professionals, ensure timely discharges by efficiently managing caseloads and collaborating with healthcare providers, and maintain up-to-date knowledge of community resources and services to facilitate access for patients.

Qualifications and Experiences:
To be considered for this role, you must have strong written and verbal communication skills and a minimum of 2 years of experience in relevant teams, including experience undertaking assessments and ensuring timely discharges. Additionally, you must be registered with Social Work England, be eligible to work in the UK, and hold a full UK driving licence. Post-qualified experience and a degree level or equivalent qualification in social work are also required.
