A vacancy has arisen within the Unplanned Care Team, which has an innovative approach to nursing care. It involves quick responses to patients in their home to assess, plan, manage, and evaluate the presenting issue, prevent unnecessary hospital admissions, and help facilitate early hospital discharges. You will support the clinical band-7 with the daily leadership and management of the team and liaise with the wider multi-disciplinary team.
Main duties of the job
We are looking for a highly motivated applicant who is interested in meeting the challenge of working in the community with patients in their home setting. As part of the Unplanned care team, you will be working with a variety of disciplines within the multidisciplinary team to achieve the best outcomes for patients. You'll be working on a rotational basis as part of a seven-day service covering 24-hours-a-day with the support of agile technology to help you deliver timely, high-quality care.
The successful candidate will have excellent clinical skills, demonstrate leadership qualities, implement high standards of patient care, and ensure a positive patient experience.
