Job Advert: Qualified Social Worker (QSW) - Hospital Discharge D2A Industry: Social Care | QSW (Qualified Social Worker) The Role We're on the lookout for a dedicated Qualified Social Worker (QSW) to join our Hospital Discharge D2A (Discharge to Assess) team. This is a dynamic role designed to ensure safe and effective transitions from hospital to home or other care settings. You'll collaborate with healthcare professionals, patients, and families, supporting them through the discharge process and beyond. What You'll Be Doing ✔ Conducting comprehensive assessments of patients' social care needs upon discharge. ✔ Collaborating with healthcare teams to develop tailored discharge plans. ✔ Supporting families and caregivers with aftercare services and guidance. ✔ Coordinating community resources to facilitate smooth transitions and prevent hospital readmissions. ✔ Monitoring and reviewing discharge plans to ensure they remain effective. ✔ Attending multidisciplinary meetings to discuss complex cases and share best practices. ✔ Maintaining accurate case records in line with relevant legislation and policies. ✔ Promoting safeguarding policies and the welfare of vulnerable adults. What We're Looking For Qualifications: Degree in Social Work (or equivalent). Registered with Social Work England. Minimum of 2 years PQE, with experience in CHC (Continuing Healthcare) and DST (Decision Support Tool). Previous experience in hospital discharge or a similar role within social care is preferred. Strong knowledge of the Care Act and Mental Health Act. Skills & Competencies: ✅ Excellent communication and interpersonal skills. ✅ Strong analytical and problem-solving abilities. ✅ Ability to work independently and within a multidisciplinary team. ✅ Empathy and compassion for vulnerable adults and their families. ✅ Strong organizational skills with the ability to manage multiple cases. ✅ Knowledge of community resources and adult social care services. ✅ Proficiency in IT systems and case management software.
