Overview
We are seeking a compassionate and organised Care Coordinator to join our healthcare team. The Care Coordinator will be responsible for overseeing and coordinating patient care, ensuring a seamless experience across multiple healthcare providers and services. The ideal candidate will provide support to patients with complex medical needs, helping them understand their care plans, accessing necessary resources, and navigating the healthcare system. This role is critical in improving patient outcomes, ensuring quality care, and promoting a positive patient experience.
Key Responsibilities:
* Patient Assessment: Evaluate patients’ healthcare needs, review medical history, and develop personalised care plans in collaboration with medical teams.
* Care Coordination: Ensure all aspects of a patient's healthcare—appointments, treatments, follow-ups, and resources—are well-coordinated among various providers and services.
* Communication Facilitation: Act as a liaison between patients, families, and healthcare providers, ensuring all parties have clear information and instructions.
* Progress Monitoring: Track patient progress, monitor adherence to care plans, and update care plans as needed to improve health outcomes.
* Resource Management: Connect patients with available community resources, including social services, support groups, financial assistance, transportation, and other supportive services.
* Patient Education: Educate patients and their families on their care plans, medications, lifestyle changes, and strategies for disease management.
* Documentation: Maintain accurate and up-to-date patient records, documenting all care activities, communication, and adjustments to treatment plans.
* Problem Solving: Identify and resolve barriers to care, helping patients overcome challenges related to access, understanding, and affordability of care.
Qualifications:
* Education: Bachelor’s degree in healthcare administration, nursing, social work, or a related field preferred.
* Experience: Previous experience in patient care coordination, case management, or a similar healthcare role is preferred.
* Skills: Strong organisational and time-management skills, excellent communication and interpersonal skills, knowledge of healthcare systems, medical terminology, and patient care practices, ability to work with diverse populations and handle sensitive patient information confidentially.
* Attributes: Compassionate, patient-centered approach with problem-solving abilities and attention to detail.
Additional Requirements:
* Proficiency in Microsoft Office Suite and ACCESS healthcare management software.
* Familiarity with CQC regulations and other healthcare compliance standards.
* Ability to work collaboratively in a multidisciplinary team environment.
* Flexibility to handle varying patient needs and case loads.
