The Colorectal CNS will act as an autonomous practitioner and will be responsible for providing specialist nurse support to a cohort of patients. The role will involve working as part of an MDT to provide a high standard of care to our patients. The role will involve the provision of specialist advice, liaison and advocacy with appropriate health care professionals across all care settings including GP's and community clinics.
Key Responsibilities:
* To act as a key worker; delivering evidence based, personalised care through holistic assessment, care planning and evaluation of care through the running of nurse-led clinics for patients from diagnosis, through treatment and follow up. This might be via a clinic, inpatient or remote follow up setting.
* To work at an enhanced level to plan and provide clinical care and psychological support to patients with a cancer diagnosis who have complex needs.
