Job Purpose: The post holder will consult with patients presenting for minor illness assessments face to face, over the phone, and through electronic means. The focus of the role is both the delivery of evidence-based practice for patients presenting within the Acute Hub with both long-term conditions and/or minor illness and will promote and support the provision of preventative health care to the practice population. As an autonomous practitioner, you are responsible for the care delivered, demonstrating critical thinking and skills in clinical decision-making in the management of patients. You will work collaboratively within and be supported by the general practice team, especially GPs, to meet the needs of patients and support the delivery of policy and procedures.

Key Responsibilities:
1. Clinical Practice: To act as an autonomous practitioner providing clinical assessment, diagnosis (where possible), and treatment (where appropriate) for patients (Adults & Paediatrics) presenting with minor illness and advising people in respect of their continuing medical and nursing needs.
2. Support to the Acute Hub Service: To clinically triage and manage e-consultations daily as required through an electronic system. Prescribe, supply, and/or administer medication and treatments according to agreed protocols, within scope of competence.
3. Maintain accurate contemporaneous computerised clinical records of every contact, including history, examination, diagnosis, plan of care, advice given, treatment, and recommended follow-up. This includes documenting any adverse event, safeguarding child or vulnerable adult issue, or other notifiable matter.
4. Assess, plan, implement, and evaluate individual treatment plans for patients with a known long-term condition.
5. Be flexible and responsive both to the clinical needs of patients and operational needs of the service.
6. Proactively identify, diagnose, and manage treatment plans for patients at risk of developing a long-term condition as appropriate.
7. Undertake annual review of patients with chronic disease in the community as identified.
8. Undertake medication reviews in line with practice protocol as identified, face to face with the patient.
9. Initiate tests and follow through results by access to path links.
10. Work with other health care professionals to diagnose, monitor, manage, and treat long-term conditions, including non-drug-based treatment methods using a management plan, and in line with national and local policies and practice needs.
11. Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols.
12. Work with patients in order to support concordance to prescribed treatments. Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects, and interactions.
13. Work within the extended formulary and supplementary prescribing and patient group directions/patient-specific directions in collaboration with GPs and other primary care healthcare teams to meet the needs of patients.
14. Support patients to adopt health promotion strategies that promote patients to live healthily and apply principles of self-care.
15. Understand and apply legal policy that supports the identification of vulnerable and abused children and adults, being aware of statutory child/vulnerable adult health procedure and local guidance.
16. Work within policies regarding family violence, vulnerable adults, substance abuse, and addictive behaviour, and refer as appropriate.
17. Be aware of and work within the performance framework for Care Quality Commission and the National Quality Requirements.
18. Continue to develop, expand, and role model own personal clinical expertise as an autonomous/independent practitioner.
19. Contribute to the development of clinical services within the practice.
20. Contribute to the development of the practice as a learning organisation, using your advanced skills to lead, train, and support junior staff.
21. Members of the practice are interested in developing teaching and research; any interest in these areas would be welcome.
22. Contribute to the work of our practice team, undertaking clinical supervision and training of colleagues as we develop the range of staff in our team.
Administrative:
1. Participate in the administrative and professional responsibilities of the practice team.
2. Be aware of all practice clinical KPIs.
3. Be aware of responsibilities under the Data Protection Act.
4. Ensure accurate and legible notes of all consultations and treatments are recorded in the patients' notes.
5. Attend meetings as required.
6. Ensure effective communication takes place within and across teams using face-to-face, electronic, and written communication.
7. Liaise with colleagues and other professionals across the Oxfordshire Integrated Care Board to share best practices.
