Job Purpose: The post holder will consult with patients presenting for minor illness assessments face to face, over the phone, and through electronic means. The focus of the role is both the delivery of evidence-based practice for patients presenting within the Acute Hub with both long-term conditions and minor illness, and will promote and support the provision of preventative health care to the practice population. As an autonomous practitioner, you are responsible for the care delivered, demonstrating critical thinking and skills in clinical decision-making in the management of patients. You will work collaboratively within and be supported by the general practice team, especially GPs, to meet the needs of patients and support the delivery of policy and procedures.

Key Responsibilities:
1. Clinical Practice: Act as an autonomous practitioner providing clinical assessment, diagnosis (where possible), and treatment (where appropriate) for patients (Adults & Paediatrics) presenting with minor illness and advising people in respect of their continuing medical and nursing needs.
2. Support the Acute Hub Service by clinically triaging and managing e-consultations daily as required through an electronic system.
3. Prescribe, supply, and/or administer medication and treatments according to agreed protocols, within scope of competence.
4. Maintain accurate contemporaneous computerized clinical records of every contact, including history, examination, diagnosis, plan of care, advice given, treatment, and recommended follow-up.
5. Assess, plan, implement, and evaluate individual treatment plans for patients with a known long-term condition.
6. Proactively identify, diagnose, and manage treatment plans for patients at risk of developing a long-term condition as appropriate.
7. Undertake medication reviews in line with practice protocol as identified, face to face with the patient.
8. Work with other health care professionals to diagnose, monitor, manage, and treat long-term conditions, including non-drug-based treatment methods.
9. Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols.
10. Support patients to adopt health promotion strategies that promote healthy living and apply principles of self-care.
Administrative Responsibilities:
• Participate in the administrative and professional responsibilities of the practice team.
• Be aware of all practice clinical KPIs and responsibilities under the Data Protection Act.
• Ensure accurate and legible notes of all consultations and treatments are recorded in the patients' notes.
• Attend meetings as required and ensure effective communication takes place within and across teams using face-to-face, electronic, and written communication.
