We deliver high quality, holistic and child centred care to CYP who require nursing intervention, across Portsmouth and the South East Hampshire area with acute and long term conditions, complex needs, life-limiting and life-threatening illness, including CYP requiring palliative and end of life care. If this sounds like the role for you then get in touch today Main duties of the job We are working in partnership with Portsmouth Hospital University Trust to build a flexible, adaptable, confident, and competent workforce, who can support seamless pathways and improved patient journeys for children between acute or specialist hospital care and community care. As part of this exciting long rotation, we are looking for a well-motivated, enthusiastic individual to join our Integrated Community Children's Nursing service (iCCNS) in Portsmouth. Our established, passionate, and dedicated Service works collaboratively with children, young people (CYP), families and the wider multi-disciplinary team to ensure safe care is provided within the community setting wherever possible. We deliver high quality, holistic and child centred care to CYP who require nursing intervention, across Portsmouth and the Southeast Hampshire area with acute and long-term conditions, complex needs, life-limiting and life-threatening illness, including CYP requiring palliative and end of life care. This post will enable you to consolidate your Children's Nursing training whilst further developing skills and knowledge through experiencing various aspects of nursing care to CYP across a range of acute and community settings. The post will offer rotation across, Integrated Childrens Community Nursing Service (HIOWH NHSFT) and the Children's Department (PHT).
