EACH are re-establishing a full-time role, which aims to provide a focus on the transition for young people and their families who have life limited and life threatened conditions. Are you: Passionate about ensuring holistic needs, high quality standards and support is in place for a young person and their family transitioning into adult services? Enthusiastic to promote the needs of young people undergoing their transition to adulthood, through collaborative working and partnership with other service providers? Do you have: Experience and interest in working with young people and their families throughout the transition period? Experience working in palliative care? Responsibilities include: Lead on implementation of EACH's strategic plan to evolve and develop transition service across the East of England for young people moving into adulthood. To provide support, guidance and education to the young person, their family and the care team in Mental Capacity Act 2005 (MCA), Liberty Protection Safeguards, Advocacy and Best Interests.
