About Us Warwickshire County Council is pleased to announce that we are looking for an Occupational Therapy Assistant to join Warwickshire’s Reablement team. We are a well-established team of Occupational Therapists, Occupational Therapy Assistants, Reablement Officers, Reablement Supervisors, Reablement Assistants and Social Care Practitioners, who have provided years of interventions for Warwickshire Reablement customers and aim to achieve the best outcomes going through their Reablement journey. Reablement focuses on promoting independence by supporting people to do as much as possible for themselves both physically and psychologically. We do this by supporting people to continue living in their own home, preventing admissions into hospital or residential settings, and facilitating timely discharges home. We believe that by supporting people to identify and achieve their personal goals and outcomes, and by embedding personalisation, customers will have choice and control over how their support will be delivered, this will have a positive impact on health and well-being. The Role: Reablement Occupational Therapy Assistants complete assessments with people who are struggling to manage activities of daily living in their homes or who have returned home from hospital following a period of ill health. We provide people with the opportunity to work on outcomes that are important to them. You will be working within the service to respond preventatively to referrals for Social Care and support and will use interventions and a strengths-based approach to enable adults and older adults to remain in their own homes with the optimum level of independence, and to enable / support customers to achieve their desired outcomes. As you will be completing initial assessments with our customers you will risk manage and communicate to the team in regard to the customers goals. You will have guidance and support from the Occupational Therapists and Occupational Therapy Team Managers within the team. Key Requirements: To be successful in this role experience of working with a range of people with different needs promoting independence is beneficial. A passion for Occupational Therapy/Reablement and some knowledge and experience of using a range of equipment to support functional ability is desired. Computer skills are important as we use a range of databases/computer programmes to communicate and document our interventions. Excellent communication skills and the ability to work within a team as well as on your own initiative is vital to this fast-paced role. Having good time management/organisation skills is required; being solutions focussed; flexible in your approach, willing to travel across the county as needed, as well as having experience of handling confidential information with tact and discretion. The post requires you to work a shift pattern of 9 am - 5 pm and 12 pm - 8 pm Mondays to Fridays on a rota basis, including one Saturday in every 7 weeks.
