We are looking to recruit an experienced Mental Health Nurse to work as a senior Key Worker and join our established, supportive Older Adult Community Mental Health Team, based at our newly re-furbished South Bucks hub in Saffron House High Wycombe. This is a 6 month fixed term maternity leave cover position.
You should have excellent interpersonal skills, a strong understanding of Older Adults with Mental health needs, an understanding of the changes with the Community Mental Health framework and be a strong advocate for training and development of junior staff.
The team work with a large multicultural patient group of adults who are over 65, who experience severe and enduring mental illnesses.
Main duties of the job
1. Oversee a team of newly qualified Nurses, Occupational Therapists, Nurse Associates, Mental Health and wellbeing practitioners, and Support Workers, providing clinical care and operational leadership.
2. Work in partnership with agencies such as Bucks CC, Dementia Support services, Age UK, private care homes, and substance misuse services to deliver comprehensive care.
3. Provide care and interventions primarily in patients’ homes or supported living accommodations.
4. Conduct urgent mental health and risk assessments for patients with dementia and memory loss, and develop care plans in collaboration with patients and their families.
5. Participate in twice-weekly MDT meetings, incorporating patient-centered care and evidence-based practices, and working closely with doctors, social workers, nurses, occupational therapists, support workers, and psychological therapies staff

Detailed job description and main responsibilities
The Bucks South OA CMHT offers an opportunity to engage in training courses and develop senior clinical and leadership skills. The management team are committed to the well-being and development of the staff working in the team.
Services for older Adults in Bucks have multiple functions: A duty assessment service, a memory service for the diagnosis and support of people with dementia, a Crisis team and an Older Adult CMHT service. The whole Service provides care and treatment to older adults with functional and organic mental health problems. As a team, the CMHT strives to ensure that we deliver high quality, patient-centred care and that our staff are supported in their roles and encouraged to develop.

This role will require you to travel between rural community bases across Wiltshire and Somerset on a daily basis. You must therefore be comfortable with driving for work purposes and able to travel freely within the hours of the role. Essential criteria
17. A specialist knowledge and clinical expertise in community mental health in line with professional requirements
18. To have awareness of National and local issues concerning healthcare
19. To have a developed understanding of Clinical Governance
20. To understand the legal frameworks used within practice
21. To maintain professional accountability and professional registration.
Experience
Essential criteria
22. Relevant Professional qualification (RMN)
23. Substantial experience of mental health care and working across different settings which must include experience within a community mental health setting.
24. Evidence of continuing professional development
25. Providing clinical and or managerial supervision
26. Hold a Psycho-social Intervention qualification or equivalent
Desirable criteria
27. To hold an advanced professional/clinical qualification (e.g. CBT, DBT, NMP)
28. Post qualifying education e.g Msc modules, in relevant area
