Are you an experienced Adults Team Manager looking for a new challenge?
We are working with Hackney Council to appoint a Preparing for Adulthood Team Manager to join their Integrated Learning Disability Service.
The Integrated Learning Disabilities Service (ILDS) is a multi-disciplinary service comprised of social workers, psychologists, psychiatrists, nurses, occupational therapists, physiotherapists, dieticians, speech and language therapists, and business support staff. The service completes Care Act assessments and reviews of need, functional and other specialist assessments. It also provides social work and health support and interventions for adults who have a confirmed learning disability, and support for their carers. The service is fully integrated and each of the teams is multidisciplinary. There is a real opportunity to improve the outcomes and experiences for adults with a learning disability working with a range of professionals, the residents themselves and their families.
Ideally, you will have experience as a Transitions Team Manager, although Learning Disabilities and Adults Team Managers will also be considered.
This is a hybrid role requiring 2 days a week in the office and 3 days from home.
