Main duties of the job
You will be an experienced Registered Nurse who is highly motivated, dynamic and enthusiastic.
Working as a member of the Hospice multi-disciplinary team to provide a high standard of skilled, compassionate nursing care and symptom management to patients in the community setting, you will provide support to patients, families and carers.
You will have excellent communication skills and be able to respond appropriately to emotionally challenging situations. Excellent nursing skills, record keeping and medicines management are an essential part of the role and previous experience in palliative and end of life care would be an advantage, however, a comprehensive induction programme and on-going mentorship are offered.

Job responsibilities
To work as a member of the Hospice multi-disciplinary team to provide a high standard of skilled, compassionate nursing care and symptom management to patients in the community setting.
To provide support to patients, families and carers.
Main duties and responsibilities
Professional/ Clinical
1. To maintain a multi-disciplinary team approach to the individualised care of the patient and maintain high standards of nursing care.
2. To assess the needs of the patient and his/ her family taking into account his/ her physical, psychological, social and spiritual needs.
3. To work closely with all members of the Community Team involved in the patients care and agree our input and level of involvement.
4. To plan, initiate, and constantly evaluate the effectiveness of care under the direction of the Community Nursing Team and Hospice@Home Team Leaders.
5. To maintain accurate written records of care, communicating all necessary information to other team members and any Community Services involved.
6. To participate fully in the setting, implementation and monitoring of formal standards of practice and to be involved, when required, in clinical governance and audit.
7. To comply with the requirements of the Nursing and Midwifery Council regarding Professional Registration and to Post-Registration professional education and revalidation.
8. Communicate appropriately with all patients, relatives, and staff, liaise with other departments and agencies, and attend departmental and interdepartmental meetings as requested.
9. To take part in the audit process as required.
10. To participate in full rotation across all shifts.
Administrative/ Managerial
1. To maintain a caring and mutually supportive working environment for all members of staff and volunteers, being sensitive to the emotional needs of colleagues and giving appropriate support to them.
2. To participate in informal internal rotation.
3. To maintain accurate records regarding the administration and custody of drugs within the community setting. To comply with all statutory and organisational policies relating to drug administration and management.
4. To maintain high standards of safety within the home, reporting any hazardous occurrences or equipment.
5. To report all accidents, incidents, complaints, and untoward occurrences, keeping statutory and organisational records as required.
6. To exercise care and economy in the use of property and equipment.
Education
1. To assist in the teaching of new staff and volunteers so that they are able to function fully within the team.
2. To participate in the teaching and guidance of visiting professionals eg Student Nurses, Social Workers and Post Graduate Students.
3. To communicate skills learnt on secondment or study leave to other members of staff.
4. To access and positively engage in role-related e-learning as directed by line managers and Lead Nurse for Education.
Other
1. Attend all mandatory training or courses.
2. Attend any training or development required by the Hospice.
3. Read, understand and follow all Hospice policies and procedures.
4. Participate in the Annual Performance Review.
5. Positively promote, support and represent the Hospice especially within the community, and to maintain the philosophy, ethos and ambience.
