Join Our Team as an Assessment and Intervention Worker (Band 6)

We're looking for an experienced Assessment and Intervention Worker to join our dedicated Somerset Talking Therapies Service.
Based in Taunton and South Somerset, you will play a key role in delivering high-quality advice, assessments, and Step 1 interventions to patients who are referred to or self-refer to our service. You'll provide consultations not only to patients but also to referrers, including GPs and mental health services, ensuring that individuals receive the appropriate care and support they need.
Main duties of the job
Your responsibilities will include:
1. Conducting face-to-face assessments and offering Step 1 interventions to patients.
2. Consulting with teams across the service and liaising with external stakeholders to ensure seamless patient care.
3. Being part of the Duty Team, helping to screen new referrals and maintaining up-to-date knowledge of service offerings to refer patients appropriately.
4. Collaborating with secondary care mental health services, leading specialist assessments, and working closely with wider mental health teams.
5. Maintaining high standards of electronic patient records and delivering collaborative risk assessments to ensure patient safety.

Communication and Key Working Relationships
* The post holder will be regarded as a senior member of the Team and will be a role model for high professional standards of behaviour and attitude within the Team and with people using the service, other professionals and agencies.
* Support the team leader through contributing to clinical leadership within the Team.
* Develop an area of specialist practice and knowledge within the team and to provide specialist advice, information and supervision (according to level of competence and experience) for other team members. To act as a team representative and to develop and maintain links with other professionals and other agencies in relation to emotional health and wellbeing.
* Communicate with patients, colleagues, internal and external contacts in a courteous, professional and timely manner at all times by various methods, i.e. telephone, written, face to face.
* Participate in team and Trust meetings as required.
* Report any incidents by way of Trust Incident Reporting system.
Planning and Organisation
* The post holder will manage their time and workload effectively and efficiently.

Analytics
* Makes judgement when dealing with enquires, to resolve problems for patients and staff.
Responsibility for Patient / Client Care, Treatment & Therapy
* Provide an advice and consultation service to referrers to the service.
* Screen and triage referrals.
* Provide a signposting service for referrals.
* Undertake risk assessments in line with Trust and Service policies.
* Assess patients referred to the service both via the telephone and face to face.
* Offer Step One treatments as appropriate as indicated from the assessment, clinical formulation and evidence base.
* Have a detailed knowledge and understanding of the Talking Therapies Stepped Care model (Least Intervention First Time -- LIFT) in order to ensure that patients receive the right therapy, at the right place, at the right time.
* Work closely with colleagues in secondary care to ensure patients access the appropriate level of service.
* Work with service leaders to develop, implement and evaluate innovative ways of enabling patients to engage with emotional health and wellbeing interventions.
* Record clinical activity at a high standard on Electronic Patient Records -- IAPTus and Rio.
* Provide cover across the county.
Policy, Service, Research & Development Responsibility
* The post holder will keep up to date with new developments in emotional health and wellbeing and mental health practice and policy.
* The post holder will keep up to date clinical records on the Trust's Electronic Patient Record system (RIO and IAPTUS).
* The post holder will respect the confidential nature of clinical information and adhere to Trust policy.
* The post holder will work within their professional code of conduct and with Trust policies and procedures. This will support safe, quality practice. The post holder will be aware their actions reflect upon themselves, their professional and the Trust and will recognise this corporate responsibility.
Responsibility for Finance, Equipment & Other Resources
* Contribute to the effective management of team resources including accurate ordering, collection and storage of clinical material. May be required to act as authorised signatory.
* Take ownership of equipment supplied to enable job role to be fulfilled.
Responsibility for Supervision, Leadership & Management
* The post holder will engage in monthly management supervision and annual appraisal with the Team Manager.
* The post holder will consider engaging in additional professional supervision or specialist clinical supervision as agreed with the Team Manager.
* Support with welcoming, inducting, training, mentoring of new staff into the team as required -- including those helping the team on a short term basis.
* Act as a positive role model and lead by example.
* Assist with risk management.
Information Resources & Administrative Duties
* Maintain specialised databases: involves input, updating and retrieval of statistics.
* Provide data as required.
* Have a comprehensive understanding of Trust information systems/speciality systems.
Any Other Specific Tasks Required
