Job summary
Job Summary:To deliver nursing care in accordance with national and local policy and guidance and in accordance with the Nursing and Midwifery post holder will be a Registered Nurse in the community 'community nurse' who works as part of a team to deliver nursing care and treatment autonomously as delegated by and under the direction of the District Nurse.
Main duties of the job
Job Purpose:o The post holder will visit adult patients in their own home who have an identified nursing need including those with a long-term condition, complex care needs and needing care at home and the end of The post holder will use an electronic patient held record system, EMIS as a record of care and all our nurses are provided with a work Iphone and Ipad for mobile The community nurse will use a range of nursing skills including venepuncture, wound care, catheterisation, syringe drivers, end of life care and symptom control and on successfully attaining all the competencies required will be reviewed for a Band 6 Senior Community Nurse post.

We recognise that a highly skilled community nursing workforce is the cornerstone to safe patient care and avoiding unnecessary admission to hospital and have lots training and development opportunities available, alongside a career development pathway with opportunities for development at every level through to band 3 to band 8.
All community nurses work as part of a local team, closely with the local acute hospital, social care, mental health teams, GPs, and Practice Nurses and use the electronic clinical record, EMIS with a work Ipad and Iphone provided.

Education / Training / Qualifications
Essential
1. Registered Level 1 Nurse with current Nursing and Midwifery Council registration
2. Educated to diploma or level 6 / degree level in relevant area
3. Supervisor / Mentorship qualification (within the first 2 years of post)
4. Evidence of continuing professional development
5. Area of special interest in the management and treatment of long-term conditions and willing to undertake additional education in this area
