KEY RESPONSIBILITIES Improving patient flow within working hours hours, increasing the time efficiency of clinician time, Support reduced length of stays within dedicated rehab wards Reduce time spent by clinicians on managing correspondence and non-clinical activities related to the patient care on the 3 wards. You will be expected to develop links with local GP practices, colleagues within the service and local communities to ensure delivery of integrated, patient focused care, demonstrating flexibility in the role and a caring attitude towards patients. Sorting clinical tasks and prioritising for the GP in terms of actions. Signposting some post to others such as clinical pharmacist etc. Inputting and arranging clinical follow up and ensuring QOF coding is up to date for the patients before being discharged to their normal care provider. Extracting all information from clinical letters that needs coding and adding to notes Arranging appointments, referrals, tests and follow up appointments of patients Completing basic (non-opinion) forms and core elements of some forms for the GP to approve and sign. Helping the GP liaise with outside agencies e.g. getting an on call doctor on the phone to ask advice or arrange admission while the GP can continue with their consultation(s) As this is a new initiative and there is potential for change.
