You will be part of our progressive pharmacy team and contribute to the implementation of the Medicines Optimisation and Pharmacy Services strategy, striving for excellence to provide the highest standards of care for our patients. The successful applicant will join our pharmacy team working within Care of the Elderly. In addition to excellent communication and clinical skills, you will have previous experience of working within a clinical environment. You would have completed, or be near to completion, of a post-graduate diploma in clinical pharmacy in order to support your work as a specialist clinical pharmacist. You will contribute to the weekend, late night and emergency duty rotas. You will also be encouraged to undertake the Non-Medical Prescribing course if you have not already done so. Main duties of the job • To deliver a high quality specialist clinical pharmacy service on a rotational basis. • To work with the Advanced Clinical Pharmacist(s) in the specific clinical specialty area, and support them in service development and audit. • To undertake routine clinical pharmacy duties in designated areas of work, both clinical and operational and ensure compliance with medicines legislation. • To support pharmacy service areas in the department if required. This may include dispensaries, technical services and medicines information. • Departmental duties will require the individual where appropriate to take a clinical supervisory role. • To undertake the professional duties of a pharmacist within the Trusts service. • To ensure, under the guidance of the relevant pharmacy manager, appropriate supervision of pharmacy technicians, pharmacy support workers and trainee pharmacists and to provide mentoring of junior pharmacists in order to maintain strict compliance with checking and working procedures. Area of Responsibility: The Specialist Clinical Pharmacist will work closely with the Advanced Clinical Pharmacist to support the delivery of Medicines Optimisation and Pharmacy services to Care of the Elderly. This includes: • Contributing to the development and maintenance of policies and guidelines. • Liaising with all grades of medical and nursing staff over relevant aspects of the pharmacy service. Helping to implement necessary service changes as a consequence of these consultations. • Identifying, reporting on and responding to risk management issues or clinical incidents, referring to peers as necessary. • Generating and reviewing reports in order to assist Advanced Clinical Pharmacists to inform the Trust about medication usage and expenditure and/or safety issues related to medicines. • Participating in audits to demonstrate the effectiveness or identify development needs of all systems relating to the use of medicines. • Deliver, in collaboration with staff in the Medicines Information department, support for Formulary & Drug Usage Audit initiatives. There may be a requirement to cover other areas for other colleagues for example for annual leave or sickness cover. The post holder will be required to work at any location where the Trust provides services. Clinical Activities The post holder is accountable for his/her own professional actions. Guidance of senior colleagues will be available for reference if required but the post holder will have independent responsibility for all duties carried out on a day to day basis. Provide pharmaceutical care to patients at ward level in both the acute hospital and in organisations with which the Trust has a Service Level Agreement such as Intermediate Care and Community Services. This role involves the development and maintenance of care plans for patients on the unit/directorate, including: • taking a medication history and completing medicines reconciliation, • assessing patients own medicines, • reviewing medication regimens to ensure safe effective and cost-efficient use, • appropriate monitoring of treatment given during the hospital stay, • developing plans for care and education of the patient about treatment changes and the best way in which they can manage their own medicines, where appropriate through self administration schemes, • facilitating the discharge planning process and promote seamless care across the primary/secondary care interface, • facilitating implementation of local and national guidelines for medicines use, • liaising with staff over issues of storage and stock control and monitor and review stock holding, and • reporting medicines related incidents and complaints with a view to resolving problems and minimising risk. Provide evidence based pharmaceutical advice to the medical and nursing staff. This may be active - where pharmaceutical problems identified by the post holder require resolution, or passive - where doctors or nurses seek advice from the pharmacist. Carry out duties in the dispensary to help to ensure that medicines are provided in a timely fashion for in-patients and out-patients. These duties will include: • clinical and accuracy checking of prescriptions • dispensing medicines • dealing with medicines for clinical trial patients • ensuring adherence to medicines legislation and accurate record keeping especially for those medicines covered by the Misuse of Drugs Act • ensuring medicines are selected and employed in accordance with the Trust Medicines Charter and the Medicines Guide • supervising technicians and trainee pharmacists and mentoring junior rotational pharmacists • educating patients and/or their representatives about their medicines • liaising with various members of the multi-disciplinary team • inputting patient related data into computers • managing and prioritising workload to tight deadlines and ensuring urgent work is done • moving stock when necessary • where appropriate, handling money • making decisions relating to medicines stock availability. Person Specification Skills and Abilities Essential • Good written and verbal communication skills. • The post holder must be equally at ease talking to patients, medical and nursing staff of all grades in both primary and secondary care and will need to present written and verbal reports to senior hospital staff. • The post holder must have standard keyboard skills and a familiarity with routine office software - word processing, spread sheeting and PowerPoint. Qualifications Essential • Masters Degree in pharmacy obtained through four years undergraduate training. • Registered as a pharmacist with the General Pharmaceutical Council. Desirable • Membership of the United Kingdom Clinical Pharmacists Association. • Membership of the Guild of Healthcare Pharmacists. • Membership of the Royal Pharmaceutical Society of Great Britain. • Non medical prescribing or working towards a prescribing qualification. Experience Essential • Experience in pharmacy practice obtained through one year pre-registration training and examination. • A broad experience of clinical working in a number of specialty areas at a developed level. Desirable • Experience of working in a multidisciplinary team and experience in UK hospital pharmacy.
