Rochdale Adult Care is pleased to offer an opportunity for a positive enthusiastic Qualified Social Worker to join the Home First Hospital Discharge Hub, based within the Integrated Middleton Locality.
The post holder will be expected to have successfully completed the ASYE programme. Hospital Social Work requires you to be a confident, dynamic fast paced individual with excellent communication skills. You will have experience and motivation to work with adults, their carers’ and develop positive relationships with health colleagues across the hospital sites. Working within timescales and deadlines is essential for this role.
It is expected that the post holder will be a positive team player, and an enthusiastic individual who can demonstrate social work practice. You will have excellent knowledge and experience of the Mental Capacity Act, excellent knowledge and understanding of Safeguarding within an Adult Care setting. The post holder is expected to promote a strength based approach to assessments, support planning and offering advice and guidance to colleagues, service users and carers, as within the objectives of the Care Act 2014. Additionally, social workers are expected to promote the independence and well-being of service users and carers, utilising natural support and the voluntary sector, ensuring creativity with solutions to support planning.
In return you will have the opportunity to further develop and progress your social work practice and skills within a supportive and enthusiastic service, with an opportunity to undertake a variety of appropriate training and development courses.

The post holder’s must be able to demonstrate an ability to travel around the Borough of Rochdale and travel to various hospital site locations. The post is subject to a seven day working contract on a rota basis.
Interviews will be held on the 9th May.
This post is subject to satisfactory two year reference history, an enhanced DBS including a barred list check against the adult workforce, medical clearance, evidence of essential qualifications and proof of legal working in accordance with Asylum and Immigration Act 1996.
