The Hamilton Practice is seeking a motivated and approachable practice nurse, on a part-time basis for 20 hours per week, over 3 days, although this is flexible. Informal visits are welcome.
Main duties of the job
* Advise on childhood and adult vaccinations, ensuring safe administration.
* Run well-person clinics and health promotion programmes, providing holistic assessments and lifestyle advice.
* Promote healthy living and self-care principles.
* Perform holistic assessments for cervical cytology smear tests.
* Conduct venepuncture and take ECGs.
* Maintain infection control guidelines and manage stock and equipment.
* Assess, plan and evaluate treatment programmes for long-term conditions.
* Work with the MDT to manage long-term conditions using management plans.
* Support care for patients with asthma, COPD, diabetes, hypertension, cardiovascular disease, contraception, cervical cytology, immunisations, weight management, mental health and learning disabilities.
* Act as a Diabetes Specialist Nurse (DSN).
* Review patient needs holistically and provide person-centred care.
* Maintain accurate records and participate in clinical governance measures.
* Maintain effective relationships and communication with colleagues.
* Share learning and information with team members.
* Delegate tasks appropriately.

Person Specification
Qualifications
* Registered Nurse with the Nursing and Midwifery Council (NMC).
* Chronic disease management training/qualifications (e.g., Asthma, COPD, Diabetes, CVD).
* Qualified in Phlebotomy, ECG taking and review.
* Child/Adult Immunisation trained.
* Qualified in contraceptive health and cervical cytology.
Experience
* Experience with patient information systems and basic IT systems.
* Experience with patients with long-term conditions and diverse needs.
* Ability to work on own initiative.
* Flexible with working hours.
* Experience in primary care and general practice.
* Experience with SystmOne.
* Experience with audit and clinical governance processes.
