Job summary
We are seeking an experienced Senior Clinical Leader to provide exceptional clinical leadership and oversight across our care homes and HMT community services. This role will drive the implementation and monitoring of our innovative HMT Clinical and Care Practice Model & Framework, ensuring excellence in service delivery.
As a key member of the HMT Senior Leadership Team, you will lead the development and execution of the HMT Dementia Strategy, supporting the growth of complex and niche services in community settings. You will champion our vision and values, empowering care home staff to uphold defined standards of clinical and care practice while contributing to the successful delivery of the HMT Strategic Plan Priorities and Initiatives.

The role has the option to be remote with travel to HMT sites expected as required.
Main duties of the job
1. Provide senior Clinical Leadership and Oversight specifically to care homes and HMT community offering.
2. Lead the implementation and monitoring of the newly developed innovative HMT Clinical and Care Practice Model & Framework.
3. Support the delivery of the HMT Strategic Plan Priorities and Initiatives within the care homes, by developing and implementing an HMT Dementia Strategy.
4. Ensure that all care home staff captures our commitment to excellence in service, working to defined standards of practice.

Job responsibilities
1. Provide Clinical Leadership and Operational Management to the newly developed Clinical Expert for AHP role.
2. Provide Clinical Leadership relating to defining practice and care standards to the care homes clinical teams.
3. Lead for implementation and governance of Clinical and Care Professional strategy, including HMT Practice Framework, HMT Clinical & Care Model systems and processes.
4. Establishment and governance of a Clinical Leadership of Nursing & AHP Forum.
5. Provide Subject Matter Expertise Ownership of the Clinical & Care Risk Management Policy.
6. Contribute to clinical & care risk discussions by attending clinical risk related meetings.
7. Responsible for development, design & delivery of clinical & care risk training in care homes.
8. Oversee the delivery of quality nursing assessments, care plans and interventions in care homes.
9. Lead the HMT wide Clinical Supervision audit, reporting results to governance committees.
10. Provide clinical supervision, assistance with appraisals/PDP and advise on professional revalidation.
11. Maintain own clinical supervision requirements needed to revalidate NMC registration.
12. Lead the care homes transitions to better use of electronic recording systems.
13. Support, advise and guide for practice elements Infection Protection and Prevention in care homes.
14. Lead the development & recruitment of new clinical roles to support service model development.
15. Act as Lead Investigator for HR competence or conduct issues of clinical colleagues.
Person Specification
Experience
1. Extensive clinical and managerial experience of working across a range of health and social care organisations in community settings, including nursing and residential care homes.
2. Comprehensive expertise in Leading and Managing change and service improvement in a senior nursing role.
3. Experience of researching best practice (nationally, private, and public sector).
4. Extensive knowledge of specialist areas, in particular Safeguarding/MCA/DoLS.
5. Ability to demonstrate compassionate leadership.
6. Knowledge and experience in building relationships between providers of care in community settings.
7. Must be able to prioritise own work effectively and be able to direct activities of others.
8. Experience of managing and motivating a team.
9. Willing to engage with and learn from peers, other professionals, and colleagues.
10. Effective organiser, influencer, and networker.
Qualifications
1. Registered Nurse with current NMC registration.
2. Educated to masters level or equivalent level of experience.
3. Leadership or Managerial qualification.
Desirable
1. Project Management or Service Improvement qualification.
