Job overview
The opportunity has arisen for a Memory Assessment Practitioner role, working across the county to actively participate in the Memory Assessment pathway, with a new dedicated pathway being created.
This role is part of a dynamic review of how we deliver our memory services to broaden access and ensure delivery of specialist memory assessment in a timely manner. It is essential to enable timely diagnosis (of dementia) that supports access to early treatment and support, decision making and aids optimal management and life outcomes for those in need.
The successful candidate will be required to competently assess patients, completing also the planning and delivery of assessment outcomes, which includes dispersing dementia diagnoses, when made and discussing medication options where applicable.
The service is currently county-wide; however, we particularly encourage applicants from the south of the county, and the need to travel to bases countywide will frequently be required to support the service needs. This will be balanced with home-based working as the post is hybrid working.
Main duties of the job
The post currently involves undertaking remote (via video call) or clinic-based appointments following the Memory Pathway and Protocols.
1. Responsible for making the initial contact to support patients and carers in their understanding of the memory pathway.
2. Undertake detailed history taking using a holistic memory assessment framework.
3. Accurately record and document clinical discussions and assessment details on the RiO clinical system.
4. Present cases to a consultant-led multidisciplinary team to enable diagnostic decision making.
5. Support and participate in the development of integrated pathways and care packages.
6. Work collaboratively with the multidisciplinary team to encourage and ensure good working relationships.
7. Maintain up-to-date awareness of current practice relating to digital care and confidentiality.
Person specification
Qualifications
* Foundation Degree in Psychology or equivalent qualification in health and social care subject and/or qualification at Level 4 / foundation degree or above.
* Awareness of Mental Health Act legislation.
* Higher education in a health-related subject.
* Evidence of CPD.
Skills
* Therapeutic, rehabilitation and health care skills.
* Work within policy and procedure.
* Understanding of NHS and care pathways.
Experience
* Experience of working in a similar role or evidence transferable skills.
* Experience of working within a mental health setting.
* Providing physical healthcare support and advice.
* Experience of working in health and therapy teams.
* Experience working as an Assistant Psychologist.
Special requirements
* Ability to travel within the geographical work area.
