Job summary The Torbay Adult Health and Social care team have a vacancy for a Health and Social Care Co-ordinator working Monday - Friday between the hours of 08.00 & 20.00 with some weekend and bank holiday working Working in a team providing a single point of contact for community health and social care referrals. This will include talking to a variety of individuals over the phone, via emails and face to face. Collation of appropriate information to record a needs led referral or assessment, which will include applying Care Act Eligibility and identifying potential risk. To provide advice, information and signpost to other local statutory and voluntary services as well as completing full Care Act assessments of need and formulating Support Plans and acting as a Keyworker. You should have excellent communication & I.T. skills, have a knowledge and understanding of the Care Act Legislation & Eligibility and be able to multi task and prioritise effectively. Previous experience working in a Health and Social Care setting with Patients/service users and other members of the public is necessary as is the ability to write up documents and reports clearly and concisely.
