You will work as a registered nurse within an integrated team across the community, including: Community Nursing, Intermediate Care, and Community Matrons.
You must have excellent interpersonal and communication skills, a patient-centered approach to care, be highly motivated, and able to work autonomously as well as part of a team.
An ability to provide and receive complex, sensitive, and contentious information using empathy and reassurance skills is essential. You should be able to provide motivational and negotiation skills in areas where consent/co-operation is required or there may be barriers to understanding, including undertaking best interest decisions.
Responsibilities:
1. Deliver high quality care to housebound patients of Torquay as part of a multi-disciplinary team and in partnership with primary and secondary care.
2. Ensure patient comfort and well-being are paramount.
3. Achieve agreed standards of care.
4. Contribute effectively as a valued team member.
5. Other objectives to be discussed, set, and reviewed at the time of Achievement Review.

You must be able to communicate and liaise effectively within a range of settings, including:

* Hospital Staff – Sisters/Nurses/Hotel Services/OTs/Physios
* Community Staff – Health Visitors/Community Nurses/Admin Staff

The Torquay Health and Well-Being Team is a large but supportive team that enjoys mentoring team members and students and is passionate about delivering quality care to housebound patients.
