* This is a Grade 7 post role which ranges from £30060 – £33366
* 37-hour perm contract with sleep in’s expected (enhancements provided)
o £1.58- Evening hours -from 7pm-10pm
o £3.96- Night time hours- 10pm-11pm
o £3.96- Weekend hours
o £41.78- Sleep in allowance
Responsibilities:
1. Maintain a warm, caring and stimulating environment in which service users can receive good physical, emotional, social and intellectual care.
2. Act as a key worker to a number of service users, working in partnership with them and any other relevant persons to identify needs and co-ordinate/implement their individual care plans.
3. Contribute to the process of assessment, implementation, recording and reviewing of individual care plans to the standards identified in the service specification.
4. Maintain effective record systems to enable the preparation of Social Work reports for Court Reviews, Planning Meetings etc.
5. Liaise with other professional staff and relevant external people, so that appropriate resources may be identified and mobilised for the benefit of the individual.
6. Be aware of and comply with statutory requirements, group policies, equal opportunities and any other regulation and procedure. Identify problems and service deficiencies by the continuous review of service user needs and make recommendations for improvement so as to contribute to the achievement of the overall service objectives.
