DESCRIPTION: We are hiring per diem Physical Therapists to join our dynamic care team at The Tremont Rehabilitation and Skilled Care Center in Wareham! Responsibilities:
1. Provide patient/resident assessment, care planning, direct treatment, and consultation to a general rehab caseload while participating in the clinical team process.
2. Provide teaching and training to caregiver staff and supervise other therapists, assistants, aides, and students as required.
3. Screen patients/residents and evaluate them to develop appropriate care plans.
4. May perform additional assigned administrative duties like assigning and overseeing resident care activities, medical record oversight, assuring execution of physician orders, ordering supplies, etc.
5. Under a physician’s order, provide direct resident treatment in accordance with an established plan of care.
6. Contribute to team meetings and decision-making processes as defined by the program.
7. Consult with and train patients, residents, and caregivers.
8. Monitor efficiencies on an ongoing basis in collaboration with the supervisor.
9. Document activities according to the company’s clinical documentation system in a timely manner.
10. Demonstrate consistent knowledge and application of Medicare and other reimbursement regulations.
11. Communicate effectively with patients, residents, team members, customers, and supervisors.
12. Participate in clinical program development and implementation.
13. Participate in the training and orientation of other staff, students, and volunteers.
14. Participate in the interview process for new staff.
15. Participate in census building and marketing activities.
16. Participate in quality improvement activities.
17. Ensure service and operational excellence of all assigned activities to meet or exceed expectations.
18. Comply with facility Privacy Practices policies and procedures and protect residents’ individual health information.
19. Exhibit courtesy, compassion, and respect to patients, residents, families, caregivers, visitors, physicians, administrators, and co-workers.
Education & Experience: Bachelor of Science Degree, Master of Science or Doctoral degree in Physical Therapy. Appropriate licensing requirements by individual states must be maintained. Two (2) years of clinical experience preferred.
