Are you an Occupational Therapist?
We are looking for a qualified and experienced Occupational Therapist to join our team as a Case Manager in our recently acquired business, Enable Therapy Services.
As a Case Manager, you will proactively manage rehabilitation for individuals who have experienced a personal injury or illness impacting their functional capabilities.
This position offers a salary of £40,000.00 and requires a full-time commitment of 37.5 hours per week, Monday to Friday between the hours of 8am and 5pm. We are also open to considering part-time hours with a minimum of 22.5 per week, spread over 3 days.
Responsibilities:
1. Provide a professional service to individuals and others who are referred to ETS.
2. Efficiently and effectively manage your workload so that you meet the needs of your case load in a fair and equitable manner.
3. Exercise a high level of analysis and interpretation of complex clinical presentations and situations.
4. Promote best practice and evidence-based reasoning in decision making.
5. Review treatment progress utilizing clinical reasoning and relevant practice experience.
6. Maintain regular contact with individuals, to provide support and monitor progress.
7. Effectively communicate the individual’s information in relation to rehabilitation and progress, to all relevant parties.
8. Travel to attend meetings with the individual and any relevant stakeholders.
9. Provide advice and mentoring to peers regarding the case management of individuals.
10. Assist in the induction of new staff and contribute to education and training.
11. Maintain your own competency through Continuing Professional Development (CPD) activities and maintain a portfolio which reflects personal development in readiness for HCPC audit.
12. Participate in the development and implementation of new or revised practice protocols, utilizing current evidence-based practice.
13. Work as part of a team, supporting both clinical and non-clinical staff.
14. There may be occasions when you are requested to undertake duties not documented within your job description but are considered within your level of expertise.
Requirements:
1. Qualification in Occupational Therapy.
2. HCPC registered.
3. Minimum of 5 years post-qualification experience.
4. Driving license.
5. Suitable home working environment.
6. Clean DBS check at enhanced level.
7. Good IT skills with knowledge of Office 365.
8. General clinical experience (essential) e.g. Orthopaedics, Care of Elderly, MSK, Vascular and Amputations, Mental Health, Acute medical, Community Care, long-term conditions management and rehabilitation.
9. Specialist experience (Ideal) e.g. ABI, Spinal, Paediatrics, Hands and Plastics.
10. Vocational Rehab (Ideal) supporting people to rehabilitation to work fit, return to work and maintain working role.
