We are seeking a highly skilled and experienced Senior Clinical Pharmacist to join our busy GP practice. The successful candidate will report directly to the Lead Clinical Pharmacist and the Medicines Management GP Partner. You will work collaboratively with a dynamic team of pharmacists and a pharmacy technician to deliver exceptional patient care and optimize medication management.
Applicants should be registered with the General Pharmaceutical Council and have completed an undergraduate degree in Pharmacy. Applicants should be an independent prescriber.
Main duties of the job
Chronic conditions management and clinic provision in areas including hypertension, diabetes, asthma, COPD etc.
Experience in lipid management and ability to initiate/monitor cholesterol lowering medication during cardiovascular medication reviews.
Experience in managing minor ailments and undertaking clinics to respond to patient demand within this area.
Able to support the practice to achieve its QOF targets and other practice targets which involve prescribing using clinical protocols.
Support pharmacy technician/junior pharmacist with clinical and pharmaceutical problems and provide guidance on complex medicines-related queries.
Provide expertise in clinical medicines review, address public health and social needs of patients.
Liaise with community pharmacy/hospital pharmacy teams to ensure continuity of care and resolve medication queries.
Manage patients and healthcare professionals' medicine queries.
Increase quality and safety of prescribing through mechanisms such as audits and recall systems.
Work with primary care professionals to implement NICE and other evidence-based guidelines.
Run clinics where medicines are the main interventions including DOC monitoring.
Provide medicines information and training to Practice healthcare professionals and admin staff.
Act as a source of medicines information for all of the practice team and patients (e.g. around doses, side effects, adverse events, possible alternatives e.g. around out of stocks) managing medication queries.

Primary Roles and Responsibilities
Medication Reviews
* Hold clinics for patients requiring face-to-face structured medication reviews i.e. a review of the ongoing need for each medicine, a review of monitoring needs and an opportunity to support patients with their medicine taking.
* Provide a telephone support for patients with questions, queries, and concerns about their medicines.
* Undertake structured medication reviews with patients with multi-morbidity and polypharmacy and implement own prescribing changes (as an independent prescriber) and order relevant monitoring tests.
Long Term Conditions
* See patients in long term condition clinics and implement improvements to patients' medicines, including de-prescribing.
* Manage own case load, run own long-term condition clinics in conjunction with GPs and support clinical decision making when medicine management input is required.
* Review the ongoing need for each medicine, a review of monitoring needs and an opportunity to support patients with their medicines taking.
High-risk Medication Reviews
* Manage patients at risk of hospital admissions and harm from poor use of medicines.
* Undertake clinical medication reviews with patients with multi-morbidity and polypharmacy and implement own prescribing changes (as an independent prescriber) and order relevant monitoring tests.
* Attend and refer patients to multidisciplinary case meetings.
Repeat Prescribing
* Produce and implement a practice repeat prescribing policy.
* Manage the repeat prescribing reauthorization process by reviewing patient requests for repeat prescriptions and reviewing medicines reaching review dates.
* Ensure patients are booked in for necessary monitoring tests where required.
Management of Medicines at Discharge from Hospital
* Management of medicines at discharge from hospital.
* To reconcile medicines following discharge from hospitals, intermediate care, and into care homes, including identifying and rectifying unexplained changes and manage these changes without referral to a GP.
* Perform a clinical medication review, produce a post-discharge medicines care plan including dose titration and booking of follow-up tests and working with patients and community pharmacists to ensure patients receive the medicines they need post discharge.
* Working with patients, the network, and primary care teams to minimize risks through medicines optimization.
* Review national and local policy and guidance that affects patient safety using medicines, including MHRA alerts, product withdrawals, and emerging evidence from clinical trials.
* Manage the process of implementing changes to medicines and guidance for practitioners.
* Develop and put into place protocols for managing patients on high-risk medications and train admin/reception staff in these to optimize safe management of these patients.
* Design, develop and implement clinical system searches to identify cohorts of patients at high risk of harm from medicines.
Cost Saving
Make recommendations for, and manage pharmacy technicians to, make changes to medicines (switches) designed to save on medicine costs where a medicine or product with lower acquisition cost is now available.
Clinical Advice and Expertise on Medicines
* Answer all medicine-related enquiries from GPs and other practice staff with queries about medicines, suggesting and recommending solutions.
* Provide telephone advice for patients with questions, queries and concerns about their medicines. Providing follow up for patients to monitor the effect of any changes.
Care Quality Commission
To ensure the practice(s) are compliant with CQC standards where medicines are involved.
Training
Provide education and training to the Practice staff on therapeutics and medicines optimization.
Person Specification
Experience
* Minimum of 3 years post-qualification experience.
* Have worked in general practice for at least 1 year.
Qualifications
* Completion of an undergraduate degree in Pharmacy, and registration with the General Pharmaceutical Council.
* Mandatory registration with the General Pharmaceutical Council.
* Independent prescriber.
* Membership of the Royal Pharmaceutical Society.
