Care Team Leader - Chester and Surrounding Areas
Salary: Up to £26,000 per annum

Role Overview: Care Team Leader
As a Care Team Leader, you will play a key role in delivering exceptional care services. Your responsibilities will include:
1. Meeting and assessing new service users to understand their needs.
2. Conducting client reviews and risk assessments to ensure high standards of care.
3. Handling queries and maintaining effective communication with the office team.
4. Supervising, mentoring, and supporting field staff to ensure quality care.
5. Upholding and enforcing high standards of care in line with company values, code of conduct, and ethos.
Key Requirements:
1. NVQ/QCF Level 2 or Level 3 Diploma in Health and Social Care.
2. At least 2 years of experience in the health and social care sector.
3. A Full UK driving license and access to your own vehicle.
4. Strong communication skills and the ability to develop others’ knowledge and skills.
5. A proactive approach to improving customer satisfaction.
6. Ability to build and maintain effective relationships with staff, colleagues, and stakeholders.
7. Good IT skills and a willingness to participate in on-call and backup services during out-of-hours periods.

Working Hours:
Full-time hours, including on-call services as required. The frequency of on-call duties will be discussed during the interview.
