We are seeking a registered social worker who has recent clinical experience in a community or relevant mental health inpatient setting to join a multi-disciplinary team. You will be responsible for supporting key workers within the MDT within the Personalised Care Framework with Adult Social Care input, including assessments, reviews, evaluations, and implementing adult social care support and maintaining links with adult social care colleagues. Within the role a nominated social worker supervisor can be sought to support your practice. You will have access and be invited to the social worker peer supervision group.
