Main duties of the job
* To provide a holistic patient assessment and structured carer interview.
* Responsible for completing specialist initial assessment and interpretation of results to provide a baseline for future treatment/care.
* To work autonomously and show initiative when planning clients care.
* To deliver diagnosis and offer expert advice regarding the diagnosis and treatment and provide education/support to both clients and carers.
* To provide education on the different kinds of dementia and how to manage the symptoms of dementia.
* To monitor and evaluate patient response to treatment, with autonomy to recommend and implement changes as required.
* To work as a supplementary/independent prescriber according to Trust policies.
* Autonomy to accept/re-refer/reject referrals to the service as appropriate.
* Autonomy to refer to other professionals/agencies as required.
* Required to visit clients and carers in their own home environment to complete initial assessments, reviews and treatment around care plans.

Experience
* A minimum of 6 months paid experience as Community Mental Health Nurse within the last 3 years.
* Comply with NMC Professional Registration and keep up to date with current nursing practices.
* Ensure accurate records are kept in accordance with CHS policy and NMC guidelines.
* Undertake regular mandatory training.
