About the role
In Swansea, we aim to provide adults and their carers with the right support, at the right time, by the right person. Our approach follows the Principles of the Social Services and Well Being Act (Wales), which focuses on working with people, in partnership, to meet the clients' needs and prevent issues from escalating.
We are looking for a dynamic candidate who has excellent organisational skills, is motivated, passionate, enthusiastic about the role, and committed to working alongside Swansea residents to live well and safely in our community.
This is a fast-paced role where you will assess, plan, implement, and evaluate care plans. Your passion for social work will enable effective integrated working with vulnerable adults in Swansea, their families, groups, and communities, to promote independence and social inclusion.
